- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black mold-like build up in the top interior of the ice machine.
- 36-06-4:Basic - Cardboard used on floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first. Observed cardboard being used under the rubber mats in the kitchen area.
- 36-11-4:Basic - Floors not maintained smooth and durable. Observed unsealed floors in the walk in cooler.
- 14-69-4:Basic - Ice buildup in reach-in freezer/cooler and/or walk-in freezer. Observed ice buildup up in the reach in cooler in the bar area.
- 14-33-4:Basic - Reach-in cooler shelves with rust that has pitted the surface. Observed rusted shelves in the reach in cooler on the cook line.
