Employee stated product was in service less than four hours, but lid had been open. - 29-42-4:High Priority - Vacuum breaker missing at mop sink on fitting/splitter added to mop sink faucet. Device is present but is attached at faucet, not at hose side of splitter. Educated operator on proper placement of device when a splitter is present.
