- 10-06-5:Basic - In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed shredded cheese scoop handle touching food in flip-top cooler. Person in charge removed handle from food. Corrected On-Site
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed splash zone next to nozzles on soda fountain machine soiled with black/green mold like substance. Repeat Violation
- 16-48-4:Basic - Old food stuck to clean dishware/utensils. Observed clear food containers on drying rack with old food stuck to container.
- 03D-01-5:High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. rice x 2 started at 1400 (80F - Cooling) 1550 immediately asked person in charge to place rice in ice bath to rapidly chill past 70F. Pans were uncovered 2 inch deep and on speed rack no obvious indications why cooling was not being met. Retested rice at 69F-70F by 1600. Corrected On-Site
