- 16-48-4:Basic - Old food stuck to clean dishware/utensils. Observed lettuce stuck to clean dishware in dishware storage area. Also, several containers has old label residue.
- 02D-01-5:Basic - Working containers of food removed from original container not identified by common name. Powdered seasonings in unlabeled containers. Person in charge began labeling during inspection. **Corrective Action Taken**
- 12A-29-4:High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee pick up items off of the floor, and pull up pants with gloved hands, remove gloves, put on new gloves and handle cheese without hand wash. Spoke to employee during inspection.
- 28-12-4:High Priority - Establishment operating with changed menu and/or increased seating without septic system approval for such change. Establishment is licensed for 32 seats, has 30 inside, 12 outside. Is on municipal water and sewer. Observed signature from Fire Jurisdiction. Repeat Violation
- 35A-02-6:High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed three flying insect in kitchen near mop sink.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Butter blend contains real butter held at room temperature at 74F. Person in charge states has been out of temperature for less than 4 hours. Repeat Violation Admin Complaint
- 03B-01-6:High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Meatballs in far end steam table at 90-102F. Person in charge states were heated in microwave prior to placing on steam table. Will reheat in microwave as they have been out of temperature for less than 4 hours.
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Debris buildup on toaster rollers.
