  • 12A-12-4:High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Observed employee with gloved hands place another pair of gloves over initial gloves, retrieved and cracked raw shell eggs onto the grill. Employee removed outer gloves by touching the outer exterior surface of the outer glove near wrist area. Employee discarded outer glove then took bun and placed lettuce and tomato on the bun. Company procedures state to insert two fingers inside the second poly glove above the back of wrist.
  • 29-42-4:High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker installed at main faucet, splitter added. Missing vacuum breaker on non-chemical side with attached hose.
  • 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Nozzle in/around ice cream machine has black build up. Cutting board at prep area has black staining.
