Employee loaded dirty dished onto dish rack then began to put away clean pans from the dishwasher without washing hands first. - 12A-20-4:High Priority - Employee washed hands with no soap. While inspecting women's restroom, employee used the restroom and washed hands with no soap. Inspector asked employee to wash hands and use soap. Employee washed hands again with no soap. Manager then came to restroom area and asked employee to wash hands with soap. Employee used the soap. **Corrected On-Site**
- 22-45-4:High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee bring pizza pan to the dish room, washed the pan with a scrub brush and soap, rinsed the pan and then took a towel hanging on the shelf by the dishmachine and dried the pan and brought the pan out front and placed in stack of clean pizza pans. Inspector notified manager, employee returned pan to the dish room for sanitizing.

On buffet line, prepared pizzas (105F/105F/114F/128F - Hot Holding) and cheese bread (113F - Hot Holding). Per manager all pizzas placed on buffet line between 5-10 minutes prior. Manager voluntarily discarded all pizzas and cheese bread. **Warning**
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Ice machine deflector plate front and back has black mold-like substance. Lever for ice chute on dining room drink machine has black and pink slime substance. **Repeat Violation**
