  • 38-01-4:Basic - Light shield damaged/in disrepair. In the service area there are two unshielded lightbulbs hanging from the ceiling.
  • 16-37-1:Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. There us no chemical test kit for quaternary sanitizer in the triple sink.
  • 02C-02-5:Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In the make table cooler there is tomato sauce, cheese sauce, shredded cheese with no date marking.
  • 45-05-4:Portable fire extinguisher missing from its designated location. For reporting purposes only. There is a fire extinguisher missing next to the fuse panels.
