31A-11-4:Intermediate - Handwash sink used for purposes other than handwashing. In the server prep area observed container of blender and a wet rag inside of the handwashing sink. Discussed with operator handwashing sink are only for handwashing to prevent/reduce cross contamination. Operator removed all items from handwashing sink. Corrected On-Site
02B-01-5:Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. In the menu there is a dish "ceviche" with raw seafood that is not marked indicating the consumer advisory. Operator stated dish is no longer being served and will be removed when new menus are printed.
02C-02-5:Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In the stand alone reach in cold holding unit near the entrance of the kitchen area, observed rice and beans without a date mark. Operator stated foods were prepared two days prior to inspection. In the freezer next to the cook line, observed cooked pork without date mark, operator stated food was prepared three days prior to inspection. Discussed with operator proper date marking.
