Observed several Commercially processed reduced oxygen packaged of salmon stored in the reach in cooler, located at preparation area. Manager removed salmon out of the package. **Corrective Action Taken**
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed standing dessert reach in freezer gaskets soiled, located at dry storage area. Employee cleaned it. Corrected On-Site
- 36-34-5:Basic - Observed ceiling tiles/vents soiled with accumulated mold-like substance, located at dishwasher area.
