  • 06-09-1:Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed several Commercially processed reduced oxygen packaged of salmon stored in the reach in cooler, located at preparation area. Manager removed salmon out of the package. **Corrective Action Taken**
  • 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed standing dessert reach in freezer gaskets soiled, located at dry storage area. Employee cleaned it. Corrected On-Site
  • 36-34-5:Basic - Observed ceiling tiles/vents soiled with accumulated mold-like substance, located at dishwasher area.
  • 22-16-4:Basic - Observed standing reach -in cooler interior have accumulation of soil residues, located at preparation area. Manager cleaned it. Repeat Violation
