  • 22-20-5:Basic - Accumulation of black/green mold-like substance in the ice chute of the soda machine.
  • 14-01-5:Basic - Bowl or other container with no handle used to dispense food. Observed in a container with rice at the front counter. The cook removed it.
  • 08B-38-4:Basic - Food stored on floor. Observed a case of cookie dough on the floor in the walk-in freezer. Observed plastic water bottles and other drinks in plastic bottles on the floor at the kitchen entrance.
  • 12A-07-5:High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed the cook go outside come back put on gloves and prepared sandwiches.
