- 40-06-5:Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On the shelf of the office area, dingle service napkins and pizza boxes store under tool kit and employee speaker.
- 14-11-5:Basic - Equipment in poor repair. Reach in freezers opposite the warewashing area, one door with a torn door gasket and one reach in freezer with ice build up.
- 42-01-4:Basic - Wet mop not stored in a manner to allow the mop to dry. Wet mop head stored in mop bucket by prep area.
- 11-27-4:Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. At time of inspection one employee present with food handler training through Pizza Hut, all other employees present have food manager certification. Operator unable to provide certificates or employee roster for all food handler trainings completed.
