- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine by office has a few spot of black mold-like substance. Employee cleaned interior of ice machine and removed all black spots. **Corrected On-Site**
- 24-08-4:Basic - Equipment and utensils not properly air-dried - wet nesting. By fruit washing sink, plastic pans stacked before properly air drying.
- 14-11-5:Basic - Equipment in poor repair. On cook line, stand up reach in freezer gaskets are torn on both doors.
- 36-73-4:Basic - Floor, walls and/or ceiling soiled/has accumulation of debris. Floor under ice machine by office and under fryers on cook line have build up of debris. Also, ceiling tiles above reach in freezer on cook line has dust build up. **Repeat Violation**

- 12A-12-4:High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Employee handled raw liquid egg carton to make eggs with no gloves on and immediately after pouring liquid egg into cooker handled clean equipment to put hash brown into to go container for to go order. Manager coached employee on proper handwashing procedures. Employee washed hands correctly. **Corrective Action Taken**
