- 05-09-4:Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed : the walk-in cooler and walk-in freezer did not have internal Ambient air thermometers.
- 35B-03-4:Basic - Outer openings not protected with self-closing doors. Observed : the back door self closer was not attached.
- 29-42-4:High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed : the vacuum breaker on the splitter at the mop sink is located on the chemical side. The side not connect Ted to the chemical system needs a back flow prevention device.
