- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the hood filters over the cook line have build up of grease. ALSO, the bump bars and tong handles have an accumulation of food debris.

- 36-10-4:Basic - Worn, torn and/or soiled floors/carpeting. Observed: the far end of the bar area floor has an accumulation of debris.

- 10-07-4:Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed: an ice cream scoop in standing 67F water. The water was emptied, and a fresh container was brought out to hold the scoop. Corrected On-Site
- 12A-09-4:High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed: cook staff open refrigerated drawer, pick up raw chicken with tongs that had been laying in the pan of raw chicken, and then touch cook line handles for burger press. The manager then had the staff member remove gloves and wash up before putting gloves back on. The tongs were moved to a clean, empty pan. Corrected On-Site
