Observed: prep staff adjust glasses and nose, then continue with the same gloves on working on the prep line. The manager redirected the employee, who then washed hand sanitizer added gloves. **Corrected On-Site**

Observed: lack of a vacuum breaker on the spout at the mop sink, ALSO, the mop sink now has a 4 way splitter, only one has a vacuum breaker and that one is attached to the chemical system.
