- 14-69-4:Basic - Ice buildup in reach-in freezer far back cook line, large chest freezer middle storage room.
- 12A-10-4:High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed cook touch hair and then don gloves and begin preparing food no hand wash. Spoke with operator regarding proper handwashing, operator washed hands properly before continuing food prep. **Corrective Action Taken**
