- 14-01-5:Basic - Bowl or other container with no handle used to dispense food. Paper cup observed stored in the "multigrain seasoning"
- 33-12-4:Basic - Dumpster rusted out on bottom, allowing liquid to leak onto the concrete pad.
- 12B-02-4:Basic - Employee eating in a food preparation or other restricted area. Observed open bag of "Takis" and an opened, half-eaten snack bar observed on the counter where flavored coffees are prepared. Numerous employee beverages observed on prep counters.
- 08B-49-4:Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed personal banana, plastic personal food container stored commingled with food for sale to the public in the walk-in cooler. Corrected On-Site
- 16-46-4:Basic - Old labels stuck to food containers after cleaning. Observed ice scoop stored by the large ice machine with label still attached. All labels should be removed to provide a smooth and easily cleanable surface and to not allow bacterial growth.

Vacuum breaker is required on the hose side of the "Y" connection at the mop sink. This is a repeat violation from 9/16/2021 and 7/19/2022. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator.
