- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Underside of soda machine around nozzle and ice chute has a build up of syrup debris and lime scale.
- 12A-17-4:High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. 2 separate employees rubbed hands together for only a few seconds at hand wash stations on make line.
- 12A-25-4:High Priority - Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee used bare hands to grab their shirt and wipe off mouth then put gloves on and continued to make sandwiches without washing hands before putting on gloves.
