Observed Yellowfin Tuna and Atlantic Salmon in the walk in cooler not removed from packaging and no longer frozen, operator has an employee removing from the packaging. **Corrective Action Taken**
- 14-09-4:Basic - Cutting board has cut marks and is no longer cleanable. Observed a cutting board in the prep area that is grooved in the kitchen.
- 14-11-5:Basic - Equipment in poor repair. Observed reach in cooler in the middle of the kitchen the gaskets are torn on both sides.
