- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed a black mold like substance on the ice drop tray inside the ice machine.
- 35B-01-4:Basic - Exterior door has a gap at the threshold that opens to the outside. Observed a 2" gap on the bottom right side of the back door.
- 36-68-5:Basic - Hood ventilation system inadequate as evidenced by grease accumulation on walls/ceiling. Observed grease dripping down the walls of the hood system over wok station.
- 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed heavy ice build up on condenser unit in walk in freezer.
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood filters soiled with grease buildup over cooking area.
