  • 06-09-1:Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. On line in reach in cooler; 4 tuna and 6 mahi completely thawed in sealed bags. In walk in cooler there are 10 mahi with bags cut open. Advised chef the must come out of bags.
  • 36-03-4:Basic - Cove molding at floor/wall juncture broken/missing. Missing on right side of back door.
  • 35A-03-4:Basic - Dead roaches on premises. Observed 1 dead roach beside shelving on floor an 2 in back corner on floor. Warning
