  • 29-42-4:High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker is on the chemical hose not on the fresh water hose

Establishment had added a dish washer machine in kitchen next to 3 compartment sink. Talk to Plan review and plan review stated there was no upgrade with plan review with new dish washer machine.
