- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior white panel in ice machine has old food debris on the surface.
- 36-34-5:Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed air conditioner vent around ceiling tiles has terrible dust accumulation near cook line and dishwasher area.
- 14-11-5:Basic - Equipment in poor repair. Observed broken handle on the microwave oven located on cook line.
- 08B-38-4:Basic - Food stored on floor. Observed boxes of desserts stored on the floor inside walk in freezer.

Checked hand wash sink has no warm water, found out turned off underneath the sink. Per Person in charge notify the plumber concerning the problem, using another hand sink located in the dish machine area.
