- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed soda free style machine has build up old soda splashed on the surface.( dining room)
- 36-73-4:Basic - Floor soiled/has accumulation of debris. Observed trash and debris spilled on the floor underneath the storage shelves inside walk in cooler.
- 05-09-4:Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no thermometer inside reach in cooler used in the drive thru window
- 31B-04-4:Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash signs in Men and Women restrooms.

Inspection Details:
- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed slime like growth on the top panel of the ice machine.
- 36-73-4:Basic - Floor soiled/has accumulation of debris. Observed floors greasy near the broiler and Cook line, also food debris spilled on the kitchen floor.
- 35A-02-6:High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed three flying insects in the back prep kitchen Observed four flying insects in the back prep kitchen also one flying insect in the drive thru window, called Kathryn next unannounced inspection. Recommend Manager to notify current professional pest control service.
- 03F-02-5:High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Time mark is not correct spoke to Manager changing the time on the items on the service line. 4pm-8pm place on the label at time.
- 03B-01-6:High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Checked temperature of fish 125°f stored on the transfer holding cabinet hot foods. Recommend to reheat the item.
