- 40-06-5:Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On pizza make line, employee cell phone propped up between folded pizza boxes. Manager removed cell phone. **Corrected On-Site**
- 13-03-4:Basic - Employee with no hair restraint while engaging in food preparation. Two employees have no hair restraint. **Repeat Violation**
- 36-73-4:Basic - Floor, ceiling and/or walls soiled/has accumulation of debris. Floors under pizza make line, pizza ovens, equipment in prep area soiled with food debris. Also, multiply ceiling tiles in kitchen area and dining area stained. Also, wall behind triple sink has black stains.

- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In walk-in cooler, sausage (50F - Cold Holding); ham (50F - Cold Holding). Manager stated the door to the walk-in cooler was opened this morning while pulling out product to stock front reach in coolers. Manager moved product to freezer to get temperature down below 41F. **Corrective Action Taken**
