Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Plans have not been approved for the change of ownership. Warning - From follow-up inspection 2024-06-17: During callback inspection, discovered through speaking with Robert P and plan review that applicant checked box for no plan review and it is needed. Operator spoke with plan review at the end of May but did not realize plan review did not previously receive the plans as it was included in the original application. Operator states resubmitted plans to correct email address and also called the customer service number and was given a case # for escalation to plan review. Operator is waiting on return call from plan review to get updated on plan status. **Time Extended** - From follow-up inspection 2024-08-22: Spoke with operator via phone. States received deficiency notification on 8/19/2024 and submitted requested information from plan review on 8/21/2024. Admin Complaint - From follow-up inspection 2024-09-23: Spoke with operator, plan review department and Linda S in district office. Plans were submitted July 22,2024. Second deficiency letter sent from plan review on September 16,2024. Operator states is having difficulty reaching the plan reviewer in charge of operator's file. Reviewer not in office today but spoke with supervisor who will email reviewer to contact applicant. Advised operator to follow up tomorrow with direct call to the reviewer.
