  • 14-09-4:Basic - Cutting board has cut marks and is no longer cleanable. Observed cutting board on flip-top make table with grooves and stained in kitchen.
  • 36-30-4:Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Observed plywood supporting air conditioning unit that is not smooth or easily cleanable.
  • 10-05-5:High Priority - In-use utensil stored in unclean water at or above 135 degrees Fahrenheit. Observed spoon used to serve shrimp in container of water at 86F.
  • 03F-02-5:High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed cooked shrimp at 108F. Manager advised that shrimp is kept on time, however no time mark posted. Shrimp had been cooked 40 minutes earlier.
