- 36-32-5:Basic - Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tiles above the dishwasher were water-stained.
- 10-14-5:Basic - Ice bucket at the large ice machine was stored on floor between uses.
- 10-01-5:Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop stored in the Panko bin in dry storage had the handle in contact with the product. Flour bin had scoop with handle completely buried in the product and a single-use plastic container with no handle was also buried in the product.
