- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin on the interior deflector. Repeat violation from 9-11-24. **Repeat Violation**
- 36-32-5:Basic - Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tiles (3) missing in the kitchen.
- 12B-07-4:Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Two employee drinks on prep table where employee was working with sauces and dressings. Two employee drinks on center kitchen table where condiments are stored. Employee drink sitting on top of syrup bag-in-box.
- 10-12-5:Basic - In-use ice scoop stored on soiled surface between uses. Ice scoop at the large ice machine was stored inside a soiled pan. Ice scoops at server station soda machines stored inside soiled holsters. Notified manager on duty.

- 36-52-4:Basic - Shelves attached to walls in walk-ins not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Walk-in cooler shelves heavily rusted with peeling paint.
- 38-04-4:Basic - Ten (10) foot-candles of light at least 30 inches from floor not provided in walk-in freezer. Walk-in freezer extremely dark. Flashlight was necessary to observe the area.
