- 10-01-5:Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed handle touching rice inside container at preparation area.
- 25-06-4:Basic - Single-service articles not stored inverted or protected from contamination. Observed case of single service cups stored on floor by back exit door.
- 12A-07-5:High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee changing task then changed gloves no handwashing in between.
