- 22-16-4:Basic - Reach-in freezer interior/shelves have accumulation of soil residues. Observed reach in freezer interior soiled with old food debris accumulation, located at back of the kitchen next the walk in cooler.
- 12A-10-4:High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee touched her hair then continue to prep food without washing hands prior. Coached employee on proper hand washing techniques.
- 41-10-4:High Priority - Toxic substance/chemical improperly stored. Observed spray with sanitizers stored by a container with individually packaged sugars in a cabinet under the hand sink at front line.
- 53A-10-4:Intermediate - Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Certified manager was unable to answer minimum required hot holding temperature. Coached and reviewed basic Food Code questions with manager. **Corrective Action Taken**
- 53B-16-4:Intermediate - Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Employee was unable to answer reheating required temperature specified by the food code.
