- 16-21-4:Basic - Accumulation of debris on exterior of warewashing machine.
- 06-09-1:Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed several packages of salmon with an internal temperature of 34°F. still in the package. The manager had a cook cut the corners of the bags. **Corrected On-Site**
- 14-09-4:Basic - Cutting board has cut marks and is no longer cleanable. Observed a green cutting board in the kitchen with deep groves.
- 02D-01-5:Basic - Working containers of food removed from original container not identified by common name. Observed a large plastic container with corn meal not labeled by the pizza oven. Observed several plastic squeeze bottles with different liquids in them not labeled.
- 22-41-4:High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. **Corrected On-Site**
- 08B-02-4:High Priority - Displayed food not properly protected from contamination. Observed a tray of pizza dough on a countertop next to guest seating and path to the restroom. The cook put a lid on the container and moved it behind the line. **Corrected On-Site**
- 12A-07-5:High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed the cook remove his gloves after cooking chicken then removed his gloves and put on new gloves without washing his hands.
- 12A-09-4:High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed the dish washer touch dirty dishes and then remove clean dishes with the same gloves.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Tiramisu (52F - Cold Holding); cream (52F - Cold Holding) in a reach in cooler. As per the manager the items were placed in that reach in approximately 2 hours ago. The manager put the items in a different reach in cooler. **Corrective Action Taken**

- 29-28-4:Intermediate - Water treatment device has not been inspected or serviced according to manufacturer's instructions. Observed by the ice machine outside the back door.
