Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. At routine inspection of the MFDV I observed a trailer that is used as a support vehicle . This support vehicle is not on the approved plans from the licensing inspection. The support trailer is holding paper goods, food under refrigeration, and dry goods with 2 stand up cold holding units inside . The licensing location of the MFDV has changed to a different location The address for the MFDV POTABLE water supply and location is still at the previous licensing location.
