  • 40-06-5:Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed purse and personal items, iPhone at preparation table.
  • 16-46-4:Basic - Old labels stuck to food containers after cleaning. Observed several old labels at containers at reach in cooler and walk in cooler.
  • 42-01-4:Basic - Wet mop not stored in a manner to allow the mop to dry.
  • 12A-27-4:High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee touched the eggs mix with gloves, discarded the gloves and put on new gloves without washing hands. Operator was instructed that the hands need to be washed before put on new gloves.
