- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine with mold like substance buildup on interior right wall, ceiling and deflector plate. Also, can opener blade with debris buildup.
- 36-24-5:Basic - Hole in or other damage to wall. Hole in wall by mop sink. Also, ceiling tiles missing around hood in kitchen. Also, standing water observed on floor in kitchen. Person in charge states is due to cleaning floor.
- 31B-04-4:Basic - No handwashing sign provided at a hand sink used by food employees. Missing hand wash sign at hand wash sink by bag in box soda. Also, missing paper towels at hand wash sink in kitchen.
- 16-46-4:Basic - Old labels stuck to food containers after cleaning. Observed old labels on several containers in clean dish area.
- 32-23-4:Basic - Toilet/urinal not flushing/functioning properly. Urinal in left sided men's restroom out of order.
