- 35A-06-4:Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed 7 dead roaches on sticky trap control device under triple sink. Operator discarded sticky tape control devices during this inspection.
- 24-08-4:Basic - Equipment and utensils not properly air-dried - wet nesting. Clean stacked pans on storage shelf above triple sink unable to properly air dry
- 14-11-5:Basic - Equipment in poor repair. Handle on reach-in cooler near back entry door in disrepair. Also, gasket on same reach-in cooler torn.
- 08B-37-4:Basic - Food stored in a prohibited area. Multiple boxes of tortillas stored in trailer used as dining area that is not connected to MFDV.

- 08A-20-5:High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw beef stored over ready to eat peppers in reach-in glass door cooler in dry food storage area.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. shredded mozzarella cheese (77F - Cold Holding) in bottom of reach-in cooler below front window area. Employee left cheese out and placed back in cooler upon inspectors arrival. Operator stated he will keep cheese in bottom of reach-in cooler to chill. Also, house made pico (45-46F - Cold Holding). Observed portion cups of house made pico stored higher than chill line of reach-in cooler. Operator started to move some to lower pans to chill. **Corrective Action Taken**
- 29-34-4:High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing from threaded hose Bibb at front side of MFDV. Also, 3 Vacuum breakers missing from threaded hose Bibb with splitter connected. From front side of MFDV
- 53B-13-5:Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. One employee employed more than 60 days is currently working on getting his food handler training completed.
