- 36-73-4:Basic - Floor soiled/has accumulation of debris. Observed grease build up on floor under fryer station.
- 51-36-1:Basic - Mobile food dispensing vehicle is not self-propelled or otherwise movable from place to place. Observed all tires for food truck flat and vegetation growing around them. Food truck is also completely enclosed by fences not belonging to operator of truck. Properties around truck were sold and new fences put up.

Person in charge stated food is stored at designated commissary but waste water is removed from truck and brought to commissary.
