- 14-38-4:Basic - Food storage container/container lid cracked or broken. In bulk salt container, plastic cup handle broken off.
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple reach-in cooler and walk-in freezer gaskets have black stains. Multiple bulk storage containers have soiled exteriors.
