- 32-05-4:Basic - Bathroom facility in disrepair. In men's restroom, hole in ceiling. Manager stated maintenance worker have been working on fixing. Workers will be back later today, per manager. **Corrective Action Taken**
- 14-01-5:Basic - Bowl or other container with no handle used to dispense food. In stored container of raw grits, to go bowl used as scooper.
