  • 32-04-4:Basic - Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Restroom doors are not self closing.
  • 35A-03-4:Basic - Dead roaches on premises. One dead roach observed under ice machine.
  • 14-11-5:Basic - Equipment in poor repair. Right side handle to blue bell reach in freezer broken, no longer smooth and easily cleanable. Rust observed on top of white reach in freezer in use as prep area at time of inspection. Chip observed on end of knife in use to cut strawberries.
  • 36-73-4:Basic - Floor soiled/has accumulation of debris. Floor in walk in cooler with debris buildup. Floors sticky in main kitchen. Wall at dishmachine with debris buildup.
  • 14-69-4:Basic - Ice buildup in reach-in freezer and/or walk-in freezer. All reach in freezers have heavy ice buildup. White reach in freezer near ground line has brown colored ice buildup.
