Observed Vanilla Ice Cream - Dairy (55F - Cold Holding) at 10:20 AM in shake machine hopper. Operator stated the ice cream mix came from the walk-in cooler at 7 AM. Operator had employee remove mix to move to walk in freezer to rapidly cool and place out of order sign on machine. **Corrective Action Taken**
- 31A-09-4:Intermediate - Handwash sink not accessible for employee use at all times. Observed handwash sink to the right of the three compartment sink blocked with buckets stored in handwash sink. Operator removed. Corrected On-Site
- 31B-05-4:Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels to the left of the three compartment sink. Employee corrected. Corrected On-Site
