- 35B-01-4:Basic - Exterior door has a gap at the threshold that opens to the outside. Observed: gap at the karaoke room back door. Gap at the office back door by the walk-in cooler.
- 51-13-4:Basic - No Heimlich maneuver/choking sign posted. Observed: lack of the required choking poster.
- 25-05-4:Basic - Single-service articles improperly stored. Observed: the hamburger size clam shells are stored on the floor in the pizza cook area.
- 29-45-4:High Priority - Hose attached to faucet at three-compartment/food preparation/handwash sink eliminating required air gap. Observed: the added tubing on the 3 compartment sink faucet eliminates the required air gap.
- 41-10-4:High Priority - Toxic substance/chemical improperly stored. Observed the Grill Fry spray cleaner was stored on the front line counter next to the jar of pepper flakes.
- 29-34-4:High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed the outside faucet on the way to the drive thru lacks a vacuum breaker.
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the soda gun nozzle has an accumulation of dried debris. ALSO, the inside ceiling of the microwave has dried food debris on it. The internal slide out plate/drawer for multiple Ninja style cookers have dried debris on them. The white shroud on the working ice machine is soiled.
- 31B-02-4:Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed: lack of paper towels at the front line hand wash sink. ALSO, lack of soap at the kitchen hand wash sink.
- 53B-01-5:Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed: lack of proof of any employee training.
