- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: 2 hood filters with an accumulation of dust/debris.
- 36-10-4:Basic - Worn, torn and/or soiled floors/carpeting. Observed: the corner area by the walk-in freezer door has an accumulation of debris.
- 22-40-4:High Priority - Quaternary ammonium sanitizer in spray bottle/bucket not at proper minimum strength. Observed: fresh Bucket of sanitizer at 0 ppm for quaternary amonia. The system was checked, it is not "pulling" the concentrate.
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: in the drawer on the fry line, chicken 72°. This was quick thawed this morning, and will now be put on a flat tray in the walk-in freezer to quick chill. **Corrective Action Taken**
- 41-10-4:High Priority - Toxic substance/chemical improperly stored. Observed: 2 spray sanitizer bottles on the prep table with cut leafy greens. The bottles were put in with cleaning. **Corrected On-Site**
- 29-42-4:High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink.
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the bottom shelf of the reach-in freezer has an accumulation of debris. ALSO, the sweet tea nozzle in the dining room has accumulation.
