- 40-06-5:Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed: an employee hat stored on the shelf over the prep area on the drive thru line. The hat was placed in the office. Corrected On-Site
- 36-17-5:Basic - Floor tiles missing and/or in disrepair. Observed: multiple loose floor tiles at the fryer station.
- 23-03-4:Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the wall behind the dish wash machine is soiled. ALSO, the center mixing wand(left mixer) has an accumulation of food debris.
- 25-05-4:Basic - Single-service articles improperly stored. Observed: a case of single serve lids on the floor by the office. This was moved to a shelf in the shake area. Corrected On-Site
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: cooked pasta in the cook line reach-in cooler, 52°. The reach-in cooler was turned off to defrost this morning. Pasta was moved into the walk-in cooler. ALSO, on the left end of the cook line, hot dog, 45°. All time/temperature control for safety items moved uncovered to the walk-in cooler. Hotdog was at 41° on 9 minutes. **Corrective Action Taken**
- 41-10-4:High Priority - Toxic substance/chemical improperly stored. Observed: easy off spray cleaner on the prep table at the drive thru window. This was moved to cleaning. Corrected On-Site
- 22-02-4:Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed: multiple nozzles at the drive thru soda fountain with build up.
- 53B-10-4:Intermediate - Records/documents for required employee training do not contain all of the required information. Observed: lack of date on multiple certificates.
