- 21-04-4:Basic - In-use wet wiping cloth/towel used under cutting board. Observed: a wet cloth being used under a cutting board in the back kitchen. The owner looked on line for a substitute item to stabilize the board. **Corrective Action Taken**
- 03A-02-5:High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: chicken on a speed rack by the back door, 91-101°. These were placed in the walk-in freezer. **Corrective Action Taken**
- 29-42-4:High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink.
- 11-26-1:Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed: new prep staff lacking employee training, he does not have the Employee Reporting form.

- 50-09-4:Basic - Current Hotel and Restaurant license not displayed. Observed an expired license was posted. The current license was found on line and shown to the inspector. Patrick will print tonight and post tomorrow. **Corrective Action Taken**
- 12B-07-4:Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee water bottle stored on the cook line counter. This was moved below. Corrected On-Site
- 08B-12-5:Basic - Stored food not covered. Observed: in the walk-in cooler, chicken cooked today already at 41°, uncovered. This was then covered. ALSO, chicken cooked today still in a cooling process lacking over head protection. Plastic film was loosely added to these containers. Corrected On-Site
- 05-05-4:Intermediate - Ambient air thermometer in holding unit not accurate within plus or minus 3 degrees Fahrenheit. Observed: in the front line glass doors reach-in cooler , Ambient air thermometer reading 22F, when the Ambient air temperature was 42°.
- 02D-06-5:Intermediate - Packaged food has no English labeling. Observed the bulk seasonings are only labeled in Chinese. A cook staff only reads Spanish. Please add languages to all required items.
