Observed diced chicken (44F - Cold Holding); cooked broccoli (45F - Cold Holding) located inside walk in cooler, less than 4 hours. Advised to remove tops off of food containers to rapidly cool food items to 41F and below. **Corrective Action Taken**
- 48-01-5:No current insurance inspector boiler report or boiler certificate available for boiler. For reporting purposes only. Observed 2 boiler units at 650,000 BTU's without certification located at boiler room/ dry storage.
