- 22-20-5:Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice machine in the cube maker part of unit in need of cleaning, mold like substance present.
- 21-23-4:Basic - Establishment using sponges on cleaned and sanitized, or in-use food-contact surface. Observed wet sponge in hand wash sink and inquired what the sponge was used on, was told to wash dishes, sponges are only to be used on items that not need a sanitation surface, floors, walls, and etc.

Currently operator has a electrical issue with new water heating unit, breaker continues to flip off, not maintaining hot water on a continued flow, operator is utilizing a pour over coffee maker for hot water in 3 compartment sink at 110°. Hand wash sink water at 76-78°
