- 12B-07-4:Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Personal drinks stored improperly in multiple areas in the back prep area and on the cook line.
- 41-07-4:High Priority - Container of medicine improperly stored. Containers of medicines were improperly stored with food in multiple locations, at the coffee station, next to cheese being prepped and on dry storage shelving next to and above food.
- 12A-07-5:High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Female food handlers entered the back prep area from the walk in cooler and donned gloves with no hand wash and began prepping American Cheese.
