  • 32-09-4:Basic - A minimum of one bathroom facility is not available for public use. The restroom in the customer area is locked with a sign stating "for employee use only"
  • 08B-49-4:Basic - Employee personal food not properly identified and segregated from food to be served to the public. Employee drink and opened candy bar was sitting on top of a rack that contained pizza dough in the walk-in cooler.
  • 08B-47-4:Basic - Food not stored at least 6 inches off of the floor. Bucket of sauce attached to the sauce machine (Wunder-bar) was sitting directly on the floor.
  • 14-13-4:Basic - Food-contact surface not smooth and easily cleanable. Large, table- mounted can opener blade observed to be severely rusted.
