  • 08B-39-4:Basic - Raw fruits/vegetables not washed prior to preparation. Observed bananas being cut into before being properly washed, stickers still attached. Spoke with operator about proper washing of produce prior to being prepared.
  • 12A-17-4:High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Observed employee washing hands between task rinsing off soap after rubbing hands approximately 5 seconds.
